House Bill 1447 requires schools to post a link to the Indiana Youth Survey on their school or district website to allow parents/caregivers to view a copy of the Indiana Youth Survey questionnaires. Please use the links below to review questions. The Indiana Youth Survey is a data collection requirement for the Bipartisan Safer Communities Act: Stronger Connections Grant received by Mitchell Community Schools to fund our Youth First Social Worker for three school years. If you do not want your MJHS or MHS student to participate, please return one of the two consent forms that your student will bring home in January or February.
